Payment and Insurance

Insurance
The doctor’s service is provided to you and not an insurance company. We cannot render service on an assumption that charges will be paid by the insurance company. However, as a courtesy to our patients, we will bill your insurance company for you but we expect you to pay the co-pay / co-insurance at the time of service. If the insurance company has not paid within a reasonable length of time (45 days), we will expect you to pay the balance of your bill in full.

We participate with a number of insurance/PPO plans. It is difficult to list all of them because of the rate of change in insurance plans. We will be glad to let you know if we participate with your insurance plan.

No Insurance
Payment is due at the time of service. If you are unable to pay in full it will be necessary to set up a payment plan. If this situation is necessary for you, please make arrangements before leaving our office. We request 25% payment of your total bill as the down payment and as the monthly payment amount.
